CBD and Medications: What the Data Really Says About Interactions

If you’ve ever searched for “CBD and drug interactions,” you know the drill. The liver. Cytochrome P450. A list of drug classes. And the conclusion: ask your doctor.

All of this is generally accurate. But none of these articles answers the one question that matters to someone who takes twenty milligrams of oil at night: at what dose do these interactions actually become a concern?

The answer exists, and it’s quantified. It’s included in an expert report commissioned by the French National Agency for Medicines and Health Products Safety and written by twelve hospital pharmacologists. We’ve read it in its entirety.

The dose makes all the difference

The documented interactions come from clinical trials using 800 mg of CBD per day. A user of the oil takes between 15 and 50. That’s not a slight difference—it’s a fifty-fold difference.

How It Works, in Two Minutes

When you ingest a substance, your liver metabolizes it to facilitate its elimination. This process relies heavily on a family of enzymes called cytochrome P450, supplemented by a second group, the UGTs.

CBD uses these same pathways. By occupying them, it can slow down the metabolism of a drug that also uses them: the drug remains in the bloodstream longer, its concentration rises, and its effects are amplified. The phenomenon can also work the other way around, with certain drugs slowing down the elimination of CBD.

That’s the basic principle. The key question is to what extent this actually occurs in real life.

First correction: CYP3A4 is not the primary culprit

This is the enzyme that all articles point to. The data places it in the middle of the pack. The ANSM working group has established the following ranking of CBD’s inhibitory potency in the body:

CYP2E1 > CYP2C19 > CYP2B6, CYP2D6 > CYP3A4, CYP2C9 > CYP1A2

More troubling: the case of CYP3A4 is frankly uncertain. On the one hand, a model based on the European dossier for Epidyolex—the CBD-based medication approved for certain types of epilepsy—predicts a ninefold increase in exposure to midazolam, a marker drug. On the other hand, the actual clinical study on this same midazolam yields a ratio close to 1, meaning no interaction.

The experts do not explain this contradiction. They conclude that CBD’s inhibitory effect on this enzyme is “uncertain at high doses” and “predicted to be limited” at the doses evaluated.

Second correction: the dose makes all the difference

The working group calculated the expected hepatic exposure ratio for each cytochrome at two different CBD doses.

How to read this table. An interaction becomes clinically relevant when the ratio exceeds 2 (moderate inhibition) or 5 (strong inhibition). Below these thresholds, the effect exists in theory but does not alter clinical management.

Left column: At 800 mg per day, everything is in the red. Right column: At 120 mg, almost everything drops back down to background levels. And 120 mg per day is already the maximum dose used in the French medical cannabis trial, in patients monitored at the hospital.

The authors note that their own figures overestimate the actual situation: most drugs are eliminated through multiple pathways simultaneously, and the calculation is based on peak concentration, whereas the average over the course of a day is lower.

The comparison that puts everything into perspective

A clinical case report published in 2020 followed a patient on tamoxifen who was administered 40 mg of CBD per day. The measured effect on the treatment’s active metabolite was a 19% variation .

Now for the point of comparison. A 2019 French study measured these same concentrations based on the patients’ genetic profiles: 17.9 ng/ml in a rapid metabolizer, 3.5 ng/ml in a slow metabolizer. A factor of five, compared to a factor of 1.2 for CBD.

This genetic variability is not taken into account in any official recommendations. The report draws a striking conclusion: moderate inhibitors of this enzyme have no contraindications with tamoxifen, “so there is no reason to be more restrictive with low-dose CBD.”

In other words: your genetic makeup influences your treatment far more than your oil does.

Real interactions do exist—here are their doses

None of the above means there is never any risk. Two documented clinical cases are serious.

The second case is worth mentioning. A six-year-old girl, treated with everolimus, was started on 200 mg of pharmaceutical-grade cannabidiol per day. Six weeks later, her everolimus level had tripled. The doctors reduced the everolimus dose by half—and the level continued to rise.

Keep in mind the orders of magnitude: 200 mg, 500 mg, 2,000 mg per day, in hospitalized patients on narrow-range therapy. This is not the situation of someone taking a few drops of oil.

The risk no one talks about: additive sedation

This is where the data overturns the usual narrative.

The working group consulted VigiBase, the WHO’s global pharmacovigilance database—more than 28 million reports since 1968. A search for interactions involving cannabinoids yielded 197 cases across all countries since 2004.

Read this list: antiepileptics, benzodiazepines, opioids, alcohol. Central nervous system depressants. The reported effects confirm the mechanism—toxicity in 34 cases, drowsiness in 33, fatigue, overdose.

The report’s summary is clear: the majority of these cases involve “an increase in the neuropsychiatric effects of cannabinoids, particularly when combined with other sedative substances.”

This is not a matter of enzymatic competition. It is an additive effect.

In practice, exercise extra caution with: benzodiazepines and related drugs, sleeping pills, opioids, muscle relaxants, first-generation antihistamines, and alcohol. And in all cases, assess your alertness before driving.

One notable exception: warfarin, the only anticoagulant for which a metabolic interaction has been clearly reported—even at non-massive doses—resulting in a decrease in the INR that required an increase in dosage. If you are taking an anticoagulant, this is a conversation you must have with your doctor, without exception.

The four situations that warrant medical advice

1. Medications with a narrow therapeutic range. Anticoagulants, immunosuppressants, antiepileptics, lithium, digoxin, and antiarrhythmics. These are medications where a 20% deviation makes a difference.

2. A CBD dose exceeding 100 mg per day. This is the threshold at which the figures in the table begin to apply.

3. Liver failure. The liver does all the work. If it is struggling, all of the above considerations must be reevaluated.

4. Polypharmacy, particularly in people over 65. Five or more medications already overload the system.

The sensible approach

Start low, increase slowly. Begin with a modest initial dose and maintain it for several days before increasing.

Space out doses. A two-hour gap between CBD and your medications reduces the likelihood that both will reach their peak levels at the same time.

Keep a journal. Record the dose, time, and how you feel. It may be tedious for three weeks, but this is what will enable your doctor to provide you with helpful advice.

And above all: be consistent with how you take it. The European Epidyolex product information states that CBD’s bioavailability increases from 6.5% on an empty stomach to 14–25% when taken with a meal. A high-fat meal can therefore triple the amount of CBD that actually reaches your bloodstream.

The ANSM report draws a key conclusion from this: these variations in absorption “are likely to play a major role in the extent of interactions.”

In practical terms: 25 mg on an empty stomach on Monday and 25 mg after a heavy meal on Tuesday are not the same dose. Choose a specific time and stick to it.

What We Don't Know Yet

It would be dishonest to conclude on a reassuring note without quoting what the working group itself writes:

This assessment “is based on fragmented data, primarily in vitro, and sometimes contradictory. Clinical data from patients are lacking to corroborate these predictions.”

This is the actual state of the research. Models make predictions, clinical cases describe extreme situations, and interaction studies in consumers at real-world doses can be counted on one hand.

What the data actually reveal is more nuanced than what is commonly reported: the metabolic risk is likely overestimated at standard doses, the risk of additive sedation is likely underestimated, and a conversation with your doctor remains the only personalized solution.

Is CBD dangerous when taken with medications?
At typical doses, the risk of metabolic interaction is low based on available data. The most well-documented risk is the additive sedative effects when combined with medications that depress the central nervous system.
